If yes, your General Practitioner or Immunologist must complete an Anaphylaxis Action Plan. Once complete, please provide upload Action Plan for our records.

Please Note

Parents are to supply an EpiPen and any other emergency medication (eg antihistamine) to the College, and are to replace emergency medication prior to expiry.

Students are required to carry their emergency medication to ALL ACTIVITIES OFF CAMPUS. Medication is to be given to the supervising teacher and returned to the College Administration at the completion of the activity.

Medical Emergency Authorisation

I authorise the college to seek medical/dental attention, call an ambulance or to hospitalise my son/daughter when considered necessary. I further authorise the college that if an emergency occurs requiring surgery, anaesthetic, oxygen, blood transfusion, medication and I am unable to be contacted within a reasonable time, the college has the authority to agree to medically recommended treatment by an accredited medical practitioner on my behalf.
